Ask the client to fill the Client’s Information Sheet with her/his handwriting. Give explanation about any item of the sheet only if the client asks for. Never give or suggest any answer to any of the information heads to the client.

The Client’s Information Sheet should include following points

• Name
• Address
• Phone number (Home and Office)
• Cell Phone number
• Email
• Present Occupation
• Marital Status
• Family size
• Previous experience of hypnosis
• Purpose of the current session
• Source of information about you
• Expected result
• Signature with date

The purpose of asking the above information is to get an idea about the background of the client and the perceived problem and expected outcome from the session. Information asked about previous experience of hypnosis is important as the previous experience has definitely led the client to develop a certain kind of perception about hypnosis. All the information shall be written by the client and signed as well. This will reflect her/his commitment for the expected result by using hypnosis as a tool. Moreover handwriting reflects state of mind as it is guided by the sub-conscious mind. Analysis of handwriting will also give important information.

Source of information about yourself is only to help you to understand what modes are working to spread the information. The same may be reinforced for the future advertisement.
